Parents/Legal Guardians registering a student at Smith Elementary School
are asked to bring the following documents to begin the enrollment process:
- Photo Identification
- Proof of Residency - two (2) different sources
- Child's Birth Certificate
- Child’s SS Card
- Child's Immunization Records
- Vision, Hearing, and Dental Screening
